Holotype: USNM 141608. Paratypes: FMNH 62583 [ex USNM 141604] (1); USNM 116053 (1), 141605-07 (1, 1, 1), 141609 (1), 152976-77 [ex UW] (1, 1), 202542 [ex UW 8742] (1).

Type locality: Western Pacific, Marshall Islands, Rongelap Atoll, Lagoon coral head at Kieschiechi Island, depth 20 feet.[1]
